Understanding the Treatment

What Is Sofwave?

Sofwave is a non-invasive ultrasound platform that tightens skin, lifts and refines the brow, jawline, neck, and submental area, and stimulates the body's own collagen, elastin, and hyaluronic acid — without needles, incisions, anesthesia, or downtime. It is one of the newest generations of ultrasound aesthetic technology, and at PearlMD Rejuvenation in Toronto it has quickly become one of our most-requested non-surgical lifting treatments.

Sofwave Medical was founded in Israel in 2016 by a leadership team with a deep track record in aesthetic medical devices — including Dr. Shimon Eckhouse, the founder of Lumenis (the originator of IPL technology) and Syneron Candela, and co-founder/CTO Ariel Sverdlik, whose prior ultrasound work included UltraShape. That lineage matters because Sofwave isn't a one-off device — it's the next chapter in a long line of medical-aesthetic engineering.

Sofwave received its first FDA 510(k) clearance in 2019 for the improvement of facial lines and wrinkles. Health Canada clearance followed in February 2021, opening Sofwave to licensed Canadian aesthetic clinics — including PearlMD in Toronto. Subsequent FDA clearances have extended Sofwave's indications to eyebrow lifting (2021), submental and neck laxity (2021), short-term cellulite improvement (2022), acne scars (2023), and upper-arm laxity (2023). As of 2025, more than 865,000 Sofwave treatments have been performed across 40+ countries. FDA indications cover adults aged 22 and older.

What makes Sofwave distinct from older ultrasound-tightening systems is the way it delivers energy. Instead of focusing ultrasound into deep, narrow focal points (the way traditional HIFU and microfocused-ultrasound systems do), Sofwave uses its patented SUPERB™ (Synchronous Ultrasound Parallel Beam) technology to deposit energy in a broad, controlled layer across the mid-dermis — the precise layer where most visible tightening, lifting, and fine-line improvement actually originates. The result is meaningful collagen stimulation with a treatment experience that is faster, more comfortable, and entirely downtime-free for most patients.

Clinicians often describe Sofwave as a "bridge" treatment — sitting between injectables and surgery on the rejuvenation spectrum. It's the right tool for the patient who has progressed beyond Botox & Dysport and facial fillers alone but isn't a candidate for — or interested in — a non-surgical facelift package involving more intensive treatment. It's also commonly combined with other PearlMD treatments — Botox for expression lines, fillers for volume, Sculptra for biostimulation, microneedling for texture — to deliver a complete result.

The Science

How Sofwave Works

Sofwave's effect is built on one principle: delivering precise, controlled heat to the layer of skin most responsible for firmness — the mid-dermis — while protecting the surface and the deeper structures around it.

SUPERB™ Parallel Ultrasound Beams

The Sofwave handpiece contains seven parallel ultrasound transducers that fire together to create multiple, broad thermal coagulation zones in the mid-dermis with each pulse. Unlike older microfocused-ultrasound systems that send energy into deep, narrow focal points, Sofwave's parallel-beam design spreads energy across a planar zone at approximately 1.5 mm depth. That's where most clinically meaningful tightening — and the visible benefits of brow lifting, neck refinement, and fine-line softening — actually happens.

Precise Heat, Protected Surface

Each pulse heats the targeted mid-dermal tissue to roughly 60–70°C for 4–5 seconds. That's the precise thermal window that triggers neocollagenesis — the formation of new collagen — without damaging surrounding tissue. At the same time, the device's integrated Sofcool™ surface cooling continuously chills the epidermis throughout the treatment, protecting the skin surface and improving comfort.

Collagen, Elastin & Skin Renewal

The body recognizes those small, controlled thermal zones as micro-injuries and responds the way it does to any healing process — by producing new collagen and elastin. Over the following weeks and months, those new fibers progressively tighten and firm the dermis. The result is a gradual, natural-looking lift that develops over time — most visible at around 12 weeks post-treatment, with continued improvement up to six months.

Why "Mid-Dermal" Matters

This is the most important nuance in choosing between non-surgical tightening options. Treatments that go deeper — such as microfocused ultrasound at 3.0 or 4.5 mm — can produce stronger structural lifting but tend to be more uncomfortable and longer. Sofwave's mid-dermal precision is what makes it well-suited to patients with mild-to-moderate laxity who want visible improvement, minimal discomfort, and zero downtime. We'll cover the trade-offs in detail in the comparison section below.

Why Patients Choose It

Benefits of Sofwave

Sofwave's appeal at PearlMD comes down to a short list of practical advantages that make it especially well-suited to busy Toronto professionals and patients new to aesthetic treatments.

  • No needles, no incisions, no anesthesia. The entire treatment is delivered through a handpiece glided over the skin.
  • Zero downtime. Return to work, makeup, exercise, and your daily routine the same day.
  • FDA-cleared for facial lines and wrinkles, eyebrow lifting, lifting of lax submental and neck tissue, short-term cellulite improvement, acne scar treatment, and upper-arm skin laxity.
  • Safe for all skin types and tones. Ultrasound is pigment-independent — Sofwave is suitable for Fitzpatrick skin types I through VI, unlike many laser treatments.
  • Stimulates your own collagen and elastin. The improvement comes from your body's natural renewal — gradual, subtle, and authentic to your face.
  • Fast. Most full face and neck treatments take 30–45 minutes; smaller areas are quicker.
  • Single-session results. Most cosmetic indications need just one treatment, with optional maintenance once a year.
  • Long-lasting. Results typically last 12–24 months — significantly longer than injectable-only protocols for laxity.
  • Comfortable. Average reported pain in clinical studies sits around 3–5/10, eased further with numbing and Sofcool™ surface cooling.
  • Versatile. Treats the face, neck, décolletage, upper arms, thighs, and buttocks — making it useful for both facial rejuvenation and body-skin laxity.
  • Combines beautifully. Layers well with Botox & Dysport, filler, Sculptra, microneedling, and medical skincare for a complete plan.
  • Physician-led at PearlMD. Treatment planning and oversight by Dr. Jennifer Pearlman.
Where Sofwave Works

Sofwave Treatment Areas

Sofwave can be used across most of the face, neck, and selected body areas. Below are the areas we treat most often at PearlMD in Toronto.

  • Brow / Forehead Area — Subtle non-surgical brow lift, opens the eye area, smooths forehead lines. FDA-cleared for eyebrow lifting.
  • Lower Face & Jawline — Refines jaw definition, softens early jowling, improves the lower-face silhouette.
  • Neck — Tightens loose skin on the neck and improves laxity. FDA-cleared for lifting lax neck tissue.
  • Submental Area (Under the Chin) — Lifts and firms loose skin under the chin. FDA-cleared for lifting lax submental tissue.
  • Décolletage — Reduces the appearance of fine lines and crepiness on the chest.
  • Cheeks & Mid-Face — Improves skin firmness and texture across the cheek area.

  • Acne Scars — FDA-cleared for the treatment of acne scars; typically performed as a series of 2–3 sessions.
  • Upper Arms — FDA-cleared for the improvement of upper-arm skin laxity using the liftHD body applicator.

Regulatory & Clinical Evidence

FDA & Health Canada Clearances · Clinical Evidence

Sofwave is one of the few non-invasive ultrasound tightening platforms with regulatory clearance across the face and body in both the United States and Canada. Below is the timeline, followed by a summary of the clinical evidence behind each indication.

Health Canada & FDA Clearance Timeline

  • 2019 (USA) — First FDA 510(k) clearance for the improvement of facial lines and wrinkles. Initial U.S. commercial launch followed in 2020.
  • 2021 (Canada)Health Canada clearance, enabling Sofwave at licensed Canadian medical aesthetic clinics including PearlMD Rejuvenation in Toronto.
  • 2021 (USA) — Extended FDA clearance for eyebrow lifting and lifting of lax submental and neck tissue.
  • 2022 (USA) — FDA clearance for short-term improvement in cellulite appearance.
  • 2023 (USA) — FDA clearance for the treatment of acne scars and for the improvement of upper-arm skin laxity (using the liftHD body applicator).
  • 2024 (USA) — Sofwave platform expanded with the PureImpact™ module; total platform-wide FDA clearances now exceed nine.

Sofwave's U.S. regulatory path is via FDA 510(k) clearance (not "FDA approval" via PMA, which is a different and more restrictive pathway for novel high-risk devices). 510(k) clearance is the standard route for most aesthetic devices in this category. The Canadian pathway is Health Canada device licensing. Always look for the precise language — "FDA-cleared" is the medically accurate phrase, and the one we use at PearlMD.

Clinical Outcomes Reported in Pivotal Studies

  • Facial lines & wrinkles — In the U.S. FDA pivotal study (n=60), 86% of subjects showed measurable improvement on the Fitzpatrick Wrinkle and Elastosis Scale at 12 weeks after a single treatment; 88% showed investigator global improvement. No device-related adverse events were reported.
  • Brow lifting — In the brow/submental/neck pivotal study, approximately 79% of pre/post photos were correctly identified for brow lifting and objective brow-height improvement exceeded the prespecified threshold; investigator-assessed brow lifting was reported in 80% of subjects.
  • Neck & submental laxity — In the same study, 80% of pre/post photos were correctly identified for submental/neck lifting and investigator-assessed lifting was reported in 85% of subjects.
  • Acne scars — In the multicenter pivotal study (n=67), 97% of subjects showed improvement by blinded independent review after three monthly treatments, with mean improvement of approximately 46% from baseline severity. A 2025 peer-reviewed study in Asian patients similarly showed >90% improvement with no serious adverse events.
  • Upper-arm laxity — In the FDA pivotal summary (n=46), 93% of subjects showed improvement by blinded evaluation and 93% were rated improved or very much improved on GAIS, with no device-related adverse events.
  • Cellulite — In a 2025 multicenter trial published in the Journal of Cosmetic Dermatology (n=60 women), 91% of treated areas showed visible improvement on the Global Aesthetic Improvement Scale (GAIS) after two Sofwave sessions, with a 69% mean improvement in the Cellulite Severity Scale and 53% improvement in the Laxity Scale. Patient satisfaction was 73% and mean pain was 4.26/10.
  • Safety — Across published studies, serious adverse events have been rare to absent. The most common side effects are transient redness, warmth, mild swelling, and brief tenderness — typically resolving within hours.

Statistics summarized from FDA pivotal-study disclosures, manufacturer materials, and peer-reviewed publications including the Journal of Cosmetic Dermatology. Individual results vary based on age, skin quality, lifestyle, and the area treated. Dr. Pearlman discusses realistic expectations with every PearlMD Toronto patient at consultation.

How It Compares

Sofwave vs Ultherapy, Thermage & Morpheus8

"Skin tightening" is often used as a catch-all, but the leading non-surgical platforms work differently and excel at different things. Here's an honest breakdown of how Sofwave compares.

Treatment Energy & Depth Best For Downtime Comfort
Sofwave Parallel ultrasound beams · mid-dermis (~1.5 mm) Brow, jawline, neck, submental, fine lines, mild-to-moderate laxity, acne scars, arm laxity None Mild-to-moderate (~3–5/10)
Ultherapy Microfocused ultrasound · deeper layers up to 4.5 mm (SMAS) Deeper structural lifting in moderate laxity None to minimal Often more uncomfortable (~6–9/10)
Thermage FLX Monopolar radiofrequency · bulk dermal heating ~3–4 mm Broad-area firming & collagen stimulation Minimal Moderate
Morpheus8 / RF Microneedling Needles + radiofrequency · 0.5–7 mm Texture, pores, acne scars, deeper remodeling with some laxity Several days redness & micro-scabs Moderate; usually numbed
Lasers (CO₂, Fraxel, etc.) Light-based · surface to upper dermis Pigment, sun damage, surface texture, resurfacing Varies — days to weeks for ablative Varies by device
Surgical Facelift Excisional surgery · all tissue planes Severe laxity, heavy jowls, advanced ptosis 2–4 weeks recovery Performed under anesthesia

Sofwave vs Ultherapy

Both are FDA-cleared ultrasound tightening systems, but they work at different depths. Sofwave delivers parallel beams to the mid-dermis (~1.5 mm), while Ultherapy uses microfocused ultrasound that can reach the SMAS at 4.5 mm. For patients with mild-to-moderate laxity, brow heaviness, and submental fullness — and for patients who want a faster, more comfortable treatment — Sofwave is typically the better fit. For patients with moderate laxity who specifically prioritize deeper structural targeting, Ultherapy is often discussed.

Sofwave vs Thermage

Thermage uses radiofrequency to bulk-heat the dermis broadly. Sofwave uses ultrasound at a precise mid-dermal depth. Both are non-invasive with minimal downtime; Sofwave is generally described as more depth-selective and more comfortable, while Thermage is well-suited to broad-area firming.

Sofwave vs Morpheus8 / RF Microneedling

This is the most important comparison for most Toronto patients trying to decide between two popular options. Morpheus8 (and similar RF microneedling) uses needles plus radiofrequency to remodel collagen, refine pores, and improve texture, acne scars, and some laxity — but it requires several days of redness and micro-scabs. Sofwave is needle-free and downtime-free with a sharper focus on tightening and lifting, not surface texture. Patients prioritizing texture and acne-scar improvement often benefit from RF microneedling; patients prioritizing brow/jawline/neck tightening with no downtime typically benefit from Sofwave. Many of our PearlMD patients combine the two over time.

Can Sofwave Replace a Facelift?

To be honest: no. Sofwave is not a substitute for surgical facelift in patients with significant excess skin, heavy jowling, or advanced ptosis. It is, however, the right tool for many patients with mild-to-moderate laxity who want a meaningful improvement without surgery — and for patients who want to slow the progression of laxity in their thirties and forties.

Who It's For

Who Is a Good Candidate for Sofwave?

Sofwave works best for patients with realistic expectations and a specific set of skin concerns. You're likely a strong candidate if you have any of the following:

  • Mild-to-moderate skin laxity on the face, neck, or under the chin.
  • Early jowling or softening of the jawline definition.
  • Brow heaviness or a tired-looking upper face that you want to subtly lift.
  • Loose skin under the chin (submental fullness) without significant excess fat.
  • Fine lines or crepiness on the face, neck, or chest.
  • A desire for prevention or maintenance in your 30s or 40s — Sofwave is excellent as part of a long-term anti-aging strategy.
  • An aversion to needles, surgery, or downtime — and a preference for treatments that you can do on a lunch break.
  • Acne scars you'd like to improve without ablative laser downtime.
  • Upper-arm laxity or mild cellulite that you'd like to improve non-invasively.
  • Any skin tone. Because Sofwave uses ultrasound, it's safe across the entire Fitzpatrick skin-type spectrum.
Honest Candidacy

Who Should Avoid Sofwave?

Sofwave is impressive but isn't right for everyone. We will not recommend Sofwave at PearlMD if any of the following apply:

  • Severe skin laxity, heavy jowling, or significant excess neck skin — those concerns are better addressed surgically.
  • Pregnancy or breastfeeding — out of standard precaution, we do not perform Sofwave in these cases.
  • A pacemaker, defibrillator, or other active electronic implant in the treatment area.
  • Active skin infection, open wound, or active herpes outbreak in the treatment area.
  • Severe inflammatory or cystic acne in the area to be treated — clear the active flare first.
  • Recent dermal filler directly in the treatment area, depending on timing.
  • Metal implants in the immediate treatment area.
  • Recent use of isotretinoin (typically within the past 6 months, at clinician discretion).
  • Significant anticoagulation or bleeding disorders, depending on context.
  • Expectations of surgical facelift–level results — Sofwave is gradual and natural, not transformative in the way surgery is.

If you're not sure where you fall, a consultation with Dr. Pearlman will give you a clear, honest answer — including whether a different treatment might serve you better.

What to Expect

The Sofwave Treatment Experience

Here's exactly what your Sofwave appointment at PearlMD looks like, from the first consult through your follow-up.

1. Consultation & Candidacy Assessment

Every patient starts with an in-person consultation with Dr. Pearlman. We assess your skin, discuss your goals, review medical history and medications, photograph your starting point, and confirm Sofwave is the right treatment for what you want to achieve. If a different option (such as Sculptra, filler, or microneedling) would serve you better, we'll say so.

2. Pre-Treatment Preparation

You'll arrive with clean skin (no makeup, lotions, or oils in the treatment area). We cleanse the skin, mark the treatment zones, apply topical numbing cream if desired (~20–30 min), and then apply ultrasound gel to the treatment area.

3. Sofwave Application

The handpiece is placed against your skin and moved systematically through the treatment grid. Each pulse takes 4–5 seconds and delivers controlled heat to the mid-dermis while Sofcool™ surface cooling chills the skin throughout. You'll feel warmth and a brief prickling or zapping sensation during each pulse; most patients describe it as tolerable, and we adjust comfort measures as needed.

4. Treatment Time by Area

  • Brow alone — approximately 15–20 minutes.
  • Submental / under-chin — approximately 15–20 minutes.
  • Lower face & submental — approximately 25–35 minutes.
  • Full face — approximately 30–40 minutes.
  • Full face & neck — approximately 40–45 minutes.
  • Upper arms or thighs — approximately 60–90 minutes depending on area size.

5. Immediately After

The skin is cleaned, soothing skincare is applied, and you're free to leave. There's no required downtime — you can reapply makeup, head back to work, exercise the same day, or go straight to your evening plans. Some patients have mild redness or warmth for an hour or two; for most, the treatment is invisible from the moment they walk out.

6. Follow-Up

We typically follow up at 3 months to evaluate results. Some patients elect a second treatment at 2–3 months for enhanced effect. Most plan a maintenance treatment every 12–18 months.

When You'll See Change

Sofwave Results Timeline

Sofwave is not an instant treatment. Its strength is in gradual, natural-looking collagen renewal — improvement that develops over weeks and months and looks like a refreshed version of you, not a transformed one. Here's what to expect:

  • Immediately after: Some patients notice a subtle tightening from heat-related skin contraction. Mild redness or warmth may be visible for an hour or two.
  • 1–2 weeks: Early visible improvement begins. Skin often feels and looks tighter and smoother as initial collagen remodeling starts.
  • 4–6 weeks: Continued improvement. Brow lift, jawline definition, and submental tightening become more apparent.
  • ~12 weeks (3 months): The standard evaluation point. Most patients see their best visible results around this time, with peak collagen response.
  • 3–6 months: Continued subtle improvement as collagen remodeling completes.
  • 12–24 months: Results typically persist within this window. Many patients schedule maintenance treatments annually.

Sofwave Pre & Aftercare

Before Your Treatment

  • Arrive with clean skin in the treatment area — no makeup, lotions, oils, or sunscreen.
  • Pause retinoids and active exfoliants (glycolic, salicylic) in the treatment area for 3–5 days prior.
  • Avoid significant sun exposure or tanning in the treatment area in the week before treatment.
  • Stay well-hydrated in the 24 hours before your appointment.
  • Tell us about any new medications, recent fillers in the area, implants, recent isotretinoin use, or any skin or medical conditions.
  • Eat a light meal before your visit so you're comfortable during treatment.

Immediately After

  • You can return to work, makeup, exercise, and your normal routine the same day.
  • Mild redness, warmth, or slight swelling are normal and typically resolve within a few hours.
  • Apply broad-spectrum SPF 30+ daily — protecting your investment in your collagen.
  • Use a gentle, hydrating skincare routine for the first 24–48 hours.

In the Days & Weeks That Follow

  • Resume retinoids, peels, and active skincare after 24–48 hours (or as advised).
  • Continue daily SPF — UV exposure breaks down both existing and newly-formed collagen.
  • Be patient: results develop gradually over weeks and months. Take photos at 4, 8, and 12 weeks to see the progression.
  • Stay hydrated, eat well, sleep, avoid smoking — your collagen production responds to all of it.
  • Schedule a 3-month follow-up so we can document your result together.

How Sofwave pricing compares around the world:

Toronto / Canada (CAD): Public Toronto Sofwave pricing typically ranges from approximately $1,150 CAD for small areas like brow up to $2,950+ for full face and neck. PearlMD publishes its full Canadian price list above so you know exactly what to expect — no "call for pricing" surprises. Promotional single-area offers as low as $399 CAD exist at some Toronto clinics for marketing entry, but those are not representative of standard full-treatment pricing.

United States (USD): U.S. Sofwave pricing for full face and neck commonly lands between US$3,500–$7,000 at established practices, with prestige urban markets sitting on the higher end. Small-area pricing in the U.S. typically runs US$500–$1,200.

United Kingdom (GBP): UK Sofwave pricing starts around £1,800 at many clinics, with face-and-neck packages above that.

In context, PearlMD Toronto Sofwave pricing is competitive within the Canadian market and meaningfully more accessible than comparable U.S. urban pricing when converted — and includes the full physician-led standard of care we apply to Botox, filler, and every other aesthetic service at the clinic.

Real answers to the questions we hear most about Sofwave in Toronto — and the ones you've probably already typed into Google.

What is Sofwave?

+

Sofwave is an FDA-cleared, non-invasive ultrasound treatment for skin tightening, lifting, and collagen stimulation. It uses patented SUPERB™ (Synchronous Ultrasound Parallel Beam) technology — seven parallel ultrasound transducers — to deliver controlled heat to the mid-dermis at approximately 1.5 mm depth, triggering new collagen and elastin formation without needles, incisions, or downtime. At PearlMD in Toronto, every Sofwave treatment is physician-led by Dr. Jennifer Pearlman, MD.

How does Sofwave work?

+

Sofwave delivers high-frequency ultrasound energy through seven parallel beams to the mid-dermis, heating tissue to roughly 60–70°C for 4–5 seconds per pulse. This creates controlled thermal coagulation zones that stimulate the body's natural wound-healing response, producing new collagen and elastin over the following weeks and months. Integrated Sofcool™ surface cooling continuously chills the epidermis throughout treatment, protecting the skin while the dermis gets the therapeutic heat. The result is a gradual, natural-looking lift that develops over time — not an instant change.

What is the SUPERB technology behind Sofwave?

+

SUPERB stands for Synchronous Ultrasound Parallel Beam. Instead of focusing ultrasound into deep, narrow focal points (the way older microfocused ultrasound systems do), SUPERB deposits energy across a planar zone at the mid-dermis. Seven cooled transducers fire together to create multiple, broad thermal coagulation zones with each pulse — typically heating the target tissue to 60–70°C for 4–5 seconds. The design's purpose is broad mid-dermal coverage with maximum comfort and minimal effect on deeper structures.

What does Sofwave treat?

+

Sofwave is FDA-cleared for the improvement of facial lines and wrinkles, eyebrow lifting, lifting of lax submental and neck tissue, short-term improvement in cellulite appearance, acne scar treatment, and upper-arm skin laxity. At PearlMD Toronto it's most commonly used for non-surgical brow lift, lower-face and jawline definition, neck tightening, submental (under-chin) laxity, fine lines, and décolletage rejuvenation. It can also be used on upper arms, thighs, and buttocks with the liftHD body applicator.

Is Sofwave approved in Canada?

+

Sofwave received Health Canada clearance in 2021, and is offered at licensed medical aesthetic clinics across Canada including PearlMD Rejuvenation in Toronto. In the United States, Sofwave holds multiple FDA 510(k) clearances dating from 2019 onward across face, body, and acne scar indications. As of 2025, more than 865,000 Sofwave treatments have been performed across 40+ countries.

How much does Sofwave cost in Toronto?

+

At PearlMD, Sofwave pricing in Toronto starts at $1,150 CAD for a brow lift, $1,750 for neck and submental, $2,350 for full face or for lower face and submental, $2,750 for lower face and neck, and $2,950 for full face and neck — all in Canadian dollars, plus HST. Final pricing is confirmed at consultation based on the areas treated. By comparison, U.S. Sofwave pricing typically lands between US$3,500 and US$7,000 for face and neck, so Toronto pricing is generally favourable when converted.

Is Sofwave worth the price?

+

For the right candidate — someone in Toronto or the GTA with mild-to-moderate skin laxity who wants a non-invasive, no-downtime option for tightening and lifting — Sofwave is one of the most efficient and well-tolerated treatments available. It produces gradual, natural-looking improvement that typically lasts 12–24 months from a single session. It's not a substitute for surgery in advanced laxity, and at PearlMD we will tell you so if that's the situation. For the right candidate, the per-month cost over the lifespan of the result compares very favourably to repeated injectable-only protocols for laxity.

Does Sofwave hurt?

+

Most patients describe Sofwave as a warm, prickling sensation during each pulse. Reported pain scores in clinical studies average around 3–5 out of 10. We make the treatment as comfortable as possible with topical numbing cream (applied ~20–30 minutes before treatment), the device's integrated Sofcool™ surface cooling, and a calm, paced delivery. Most patients tolerate the experience well — it's noticeably more comfortable than many older deeper-focus ultrasound treatments like Ultherapy.

Is there downtime after Sofwave?

+

There is no required downtime after Sofwave. Most patients return to work, makeup, exercise, and their daily routine the same day. Some experience mild redness, warmth, or temporary swelling that typically resolves within a few hours. This is one of Sofwave's biggest practical advantages — Toronto professionals can fit it into a lunch break.

How long does a Sofwave treatment take?

+

A full face and neck Sofwave treatment typically takes 30–45 minutes of active treatment time, with a total clinic visit closer to 60–75 minutes including consultation, numbing, and photographs. Smaller areas like brows or submental alone take 15–30 minutes of treatment. Body areas like upper arms or thighs may take 60–90+ minutes depending on the size of the treatment zone.

When will I see Sofwave results?

+

Some patients notice subtle tightening within 1–2 weeks. Most visible results appear gradually as new collagen forms — typical evaluation is at 12 weeks (3 months), with continued improvement up to 6 months after treatment. Sofwave is a slow-build treatment, not an instant change. We recommend photographing your starting point and reviewing at 4, 8, and 12 weeks to see the progression clearly.

How long do Sofwave results last?

+

Results from a single Sofwave session typically last 12–24 months. Like every skin tightening treatment, longevity depends on age, skin quality, lifestyle factors (sun exposure, smoking, sleep, nutrition), and ongoing collagen turnover. Many PearlMD patients schedule a maintenance treatment annually to keep collagen production active.

How many Sofwave sessions do I need?

+

For most cosmetic indications — brow lift, jawline, neck and submental tightening — a single Sofwave session produces visible results. Some patients choose a second treatment at 2–3 months to enhance the effect, especially when starting with more pronounced laxity. Clinical protocols for acne scars and cellulite typically use 2–3 sessions spaced 4–6 weeks apart.

Is Sofwave safe for all skin types?

+

Yes. Because Sofwave uses ultrasound rather than light, it does not target melanin and is considered safe for all Fitzpatrick skin types I through VI. This is a meaningful advantage over many laser-based treatments, which can carry higher risk of pigmentary change in darker skin tones. Sofwave's ultrasound mechanism is skin-tone-agnostic.

What's the difference between Sofwave and Ultherapy?

+

Both are FDA-cleared ultrasound skin tightening platforms — but they work at different depths. Sofwave uses parallel ultrasound beams focused on the mid-dermis at approximately 1.5 mm, the layer most responsible for visible tightening, brow support, and fine-line softening. Ultherapy uses microfocused ultrasound at deeper levels including 3.0 mm and 4.5 mm (the SMAS) and is often pitched as a deeper structural lift. The trade-off: deeper energy delivery typically means more discomfort and longer treatment time. For mild-to-moderate laxity, comfort, and shorter sessions, Sofwave is usually the better fit. For patients prioritizing deeper structural lifting and willing to tolerate more intensity, Ultherapy may be discussed. Many Toronto patients searching "Ultherapy alternative" land on Sofwave for these reasons.

What's the difference between Sofwave and Thermage?

+

Thermage uses monopolar radiofrequency (RF) to bulk-heat dermal and subdermal tissue across a broad volume. Sofwave uses ultrasound to create precise thermal zones at a specific mid-dermal depth. Both are non-invasive with minimal downtime. Sofwave is generally described as more depth-selective and often more comfortable; Thermage is well-suited to broad-area firming. Thermage is also commonly used around the eyes and on body areas where Sofwave is not used.

What's the difference between Sofwave and Morpheus8?

+

This is one of the most common comparisons we hear at PearlMD. Morpheus8 (and similar RF microneedling) uses insulated needles plus radiofrequency at varying depths (0.5–7 mm) to remodel collagen and improve texture, pores, acne scars, and some laxity. It requires several days of redness and micro-scabs. Sofwave is needle-free and downtime-free, with a sharper focus on tightening and lifting, not surface texture. The simple rule: if your priority is texture, pores, or acne scarring, Morpheus8 is often the better fit. If your priority is brow, jawline, neck, or submental tightening with zero downtime, Sofwave is usually the better fit. Many PearlMD patients combine both over time as part of a complete plan.

Is Sofwave better than HIFU?

+

"HIFU" (High-Intensity Focused Ultrasound) is a category that includes Ultherapy and several other deeper-focus ultrasound systems. Sofwave is also ultrasound, but its parallel-beam design and mid-dermal targeting make it a different generation of the technology. Compared with traditional HIFU devices, Sofwave generally offers shorter treatment time, better-tolerated comfort, and a strong evidence base for fine lines, brow lift, neck, and submental laxity. Deeper-focus systems may produce stronger structural lifting in selected cases but at the cost of more discomfort.

Can Sofwave tighten my jawline?

+

Yes — Sofwave is particularly effective for refining the jawline contour and softening early jowling. By stimulating new collagen in the mid-dermis along the lower face and jawline, Sofwave helps tighten the skin envelope and improve definition. For patients with significant volume loss along the jaw, Sofwave is often combined with facial fillers or Sculptra for a more complete result.

Can Sofwave treat the neck and "tech neck"?

+

Yes. Sofwave is FDA-cleared for lifting lax neck tissue and is one of the most popular non-invasive options for the neck, including the increasingly common concern of "tech neck" (lines and crepiness from looking down at screens). It tightens loose skin, smooths horizontal neck lines, and improves submental laxity. The neck is one of the areas where Sofwave shines.

Can Sofwave lift the brows without surgery?

+

Yes — Sofwave is FDA-cleared specifically for eyebrow lifting. The treatment tightens the skin and supportive tissue around the brow, producing subtle elevation that opens the eye area and makes you look more rested. In manufacturer pivotal studies, approximately 80% of subjects showed investigator-assessed brow lifting. It's a popular Toronto choice for patients seeking a non-surgical brow lift without the downtime of surgery.

Can Sofwave replace a facelift?

+

Honestly — no, not in patients with significant excess skin, heavy jowling, or advanced ptosis. A surgical facelift removes excess skin and repositions deep tissues; Sofwave stimulates collagen in the mid-dermis. Sofwave is excellent for patients with mild-to-moderate laxity who want gradual, non-surgical tightening with no downtime, and for younger patients in their 30s and 40s who want to slow the progression of laxity. At PearlMD we'll tell you honestly whether Sofwave is the right fit for your goals, or whether another option (or surgery) would serve you better.

Who is not a candidate for Sofwave?

+

Sofwave is not recommended during pregnancy or breastfeeding, in patients with a pacemaker or electronic implants in the treatment area, in those with active skin infection or open wounds in the treatment area, or in those with severe cystic acne in the treatment area. Recent dermal filler in the treatment zone, metal implants nearby, recent isotretinoin use, or significant anticoagulation may also require discussion. Patients with severe sagging or heavy jowling are generally better served by surgical options. Candidacy is confirmed at consultation.

What are the side effects of Sofwave?

+

Most side effects are mild and temporary: redness, warmth, tenderness, or slight swelling for a few hours. Less common effects can include light bruising, transient pigment change, or rare blistering. Serious adverse events are uncommon in clinical studies. At PearlMD, Dr. Pearlman reviews every potential effect with you at consultation and is available if any concerns come up after treatment.

Can I wear makeup after Sofwave?

+

Yes — you can reapply makeup immediately after your Sofwave treatment. There are no microscopic skin breaks (unlike microneedling) and no requirement to keep the area bare. We just recommend using a gentle, hydrating product for the first 24 hours and applying broad-spectrum SPF 30+ before going outdoors.

Can I exercise after Sofwave?

+

Yes — you can return to your normal exercise routine the same day. Sofwave does not require activity restriction. Some patients prefer to skip a hot yoga or sauna session on day one if they feel any residual warmth, but it isn't required.

How soon before an event should I get Sofwave?

+

Because Sofwave results develop gradually over weeks and months, plan to have your treatment at least 3 months before a major event for peak visible results. That said, because there's no downtime, you can have it as close to an event as you want — results will simply continue to develop afterward. Many of our Toronto wedding-prep patients book Sofwave 3–4 months ahead alongside Botox and skin treatments.

Can Sofwave be combined with Botox, fillers, microneedling, or other treatments?

+

Yes — and it's commonly part of a combined plan. Sofwave is regularly paired with Botox & Dysport (for expression lines), lip filler and facial fillers (for volume), Sculptra (for biostimulation), microneedling, PRP injections, exosomes, and medical skincare. Many PearlMD patients build a comprehensive non-surgical rejuvenation plan that layers tightening, volume, expression-line softening, and skin-quality treatments.

How do I make my Sofwave results last longer?

+

Sofwave is an investment in your collagen — protect it. Daily broad-spectrum SPF 30+ is the single most important step; UV exposure breaks down newly-formed collagen as fast as you build it. Beyond SPF: a high-quality medical skincare routine with retinoids and antioxidants, adequate hydration and sleep, avoiding smoking, and managing stress all support ongoing collagen production. Many patients also book a maintenance Sofwave session every 12–18 months to keep the response active.

Can men get Sofwave?

+

Absolutely. Sofwave is a popular choice for men in Toronto who want non-surgical jawline definition, neck tightening, and brow rejuvenation with no downtime and no needles. The treatment is identical and the results — gradual, natural-looking improvement — suit male aesthetics well. It's an excellent gateway aesthetic treatment for men who aren't yet ready for injectables.

At what age should I start considering Sofwave?

+

Most Sofwave patients are in their 30s, 40s, and 50s — but the "right" age is really about your skin and your goals, not a number. Some patients in their late 20s use Sofwave preventatively to stay ahead of collagen loss. Patients in their 50s and 60s often combine Sofwave with Sculptra and fillers for a more comprehensive plan. The FDA indication is for adults aged 22 and older.

Will Sofwave melt or affect my dermal filler?

+

Sofwave's thermal effect is centered at the mid-dermis (~1.5 mm), while most facial filler sits deeper. In practice, Sofwave is widely considered compatible with filler — but we still ask about your filler history at consultation and plan timing carefully. If you've had recent filler in the treatment area, we typically wait a few weeks before Sofwave, or sequence the treatments thoughtfully. Sofwave and filler complement each other beautifully when planned together.
