Hormone Balance Over the Life Stages

Hormonal shifts occur across every decade of life—not only during menopause or andropause. Subtle changes often begin years earlier and can accumulate silently, influencing energy, metabolism, mood, cognition, and disease risk long before overt symptoms appear.

As we age, many hormones naturally decline due to progressive senescence of key endocrine organs, including the ovaries, testes, thyroid, adrenal glands, and thymus. This age-related hormonal decline is both a consequence of biological aging and an active driver of it, contributing to changes in body composition, immune resilience, metabolic health, and cellular repair.

Midlife hormonal transitions are particularly impactful. In women, the abrupt loss of ovarian hormones during perimenopause and menopause can profoundly affect cardiovascular health, bone density, brain function, and metabolic stability. In men, a slower, more gradual decline in testosterone and adrenal hormones—often referred to as andropause—can similarly erode vitality, muscle mass, motivation, and cardiometabolic health. The ovary is the first organ to age. Female hormonal health is complex, dynamic, and vital at all life stages. The Menopause Transition

A woman’s menopause marks the end of her reproductive years and loss of ovarian estrogen production. At menopause accelerated aging occurs and women face increased risk of most chronic diseases including cardiovascular disease, dementia and osteoporosis.

While menopause occurs on average at the age of 51 years there is a high degree of individual variation both in timing and symptoms. Menopause occurring before the age of 45 years is considered early and confers increased risk. While the definition of menopause is retrospective made 12 months after the last period has occurred, a more useful framework is to consider the menopause transition as the decade surrounding a women’s last menses and often the most symptomatic.

Perimenopause represents the stage proceeding the final menstrual period and can last 7 to 10 years. Perimenopausal symptoms may begin subtly in women in their early 40s. In the decade surrounding the menopause women will age 3-fold faster than any other and twice faster than men the same age. The age acceleration coincides with the emergence of bothersome symptoms, increased disease risk and deterioration of performance and wellbeing.

The first step to a proactive approach to the menopause transition is to identify problematic symptoms and establish an individualized plan for hormonal optimization. We recommend establishing a baseline for women by the age of 40 years to confirm reproductive stage and predict the timing of the perimenopause transition.
